Complete License Application: Print and fill out the license application form. Completed, Notarized Application with Cover Page - Applications are valid for 1 year from the day they are received at Rhode Island Department Of Health. If you are not licensed within the year you must submit a new application.
Provide proof of attending an OSHA-compliant bloodborne pathogen /Universal Precautions course by attaching a copy of your certificate to your application. You must have a passing grade of 70%.
Provide Birth Certificate (official certified copy), or if born outside the United States then you need to provide proof of citizenship, lawful alien status or legal entry(whichever applicable).
Provide a recent identification photograph of the applicant, head and shoulders, front view, approximately 2x3 inches in size;
Original BCI (Background Check) with stamp and seal directly from the RI Attorney General’s Office or Local Police Department. Applicants are responsible for all costs incurred in this process.
YOU must attach an “Interstate Verification Form” (See page 6) to each state in which you are, or ever have been, licensed as a Tattoo Artist or Body Piercer (Make copies as needed).
Note - An out-of-state license can be used as proof of experience, but can not be used in place of a Rhode Island license.
Check or money order (preferred), made payable (in U.S. funds only) to the RI General Treasurer in the amount of $62.50 and attached to the upper left-hand corner of the first (Top) page of the application. THIS APPLICATION FEE IS NONREFUNDABLE.
After you complete the application, call 401-222-2566 to schedule an appointment for your practical examination of Aseptic Tattooing Technique. Applications must be submitted by mail or drop off. The address is:

You will receive your license about 30 days after you submit your application.
Applicants seeking registration as a tattoo artist shall be required to demonstrate aseptic tattooing technique via an examination administered by the Department, to test the applicant's fitness to engage in the practice of tattooing. Such examination shall test the applicant's knowledge and practices of infection control and such other areas as may be deemed necessary, and shall be administered upon completion and review of the application.
a) Applicants who have not satisfactorily met the application requirements herein relating to the examination described above will be given two (2) additional opportunities to demonstrate tattooing technique. Failure to meet all technique requirements will result in denial of registration.
b) Applicants wishing to reapply for registration may do so after a six (6) month waiting period between attempts at the examination.
Artist permits are valid from the date of issuance and expire automatically at the end of the facility license year unless revoked earlier by the Department. Permits must be renewed annually.
