Step-wise process to get a Tattoo Artist Apprentice Card in Nevada

Fill out the Body Art Apprentice Application & Employment Verification ( Form 1B)
Print and fill out the Body Art Apprentice Application & Employment Verification. It must be signed by both apprentice and the mentor. A Body Art Apprentice Card must be issued prior to beginning the apprenticeship.

Complete a
Osha approved
Bloodborne Pathogen Course
Provide documentation of successful completion of an OSHA Bloodborne Pathogens training course, which ensures you are trained in disease prevention (including hepatitis B transmission).

Provide a Copy of a State ID
Attach a copy of your state ID that has your name and your birth date confirming that you are above 18 years of age.

Submit your Application form
Submit the required documents to -

Upon completion of at least six months training, the applicant must return to the Health District with the completed BODY ART EXPERIENCE VERIFICATION FORM (Form 2B) to upgrade to the Permanent Body Art Card.
Step-wise process to get a Tattoo Artist Card in Nevada

Fill out Body Art Experience Verification
Complete and Fill out Body Art Experience Verification form (Form 2B). Proof of previous experience must be sent to the Body Art program at BodyArt@snhd.org for approval, prior to the applicant’s appointment. Please do not schedule your test until you have the approval in hand.

Bloodborne Pathogen Course

Fill out the Body Art / Microblading Artist Application
Print and fill out the Body Art / Microblading Artist Application(Form 0). It must be signed by the Tattoo Artist.

Attach a copy of your state ID that has your name and your birth date confirming that you are above 18 years of age.

Pass the Body Art sanitation program
A score of 80% on the body art sanitation exam is required to obtain a body art or microblading card.

Pay the fee applicable
The one-time fee for a Body Art Card is $138 ($118 exam fee, $20 card fee) which is to be paid with application.
