Attach a copy of your photo ID (driver’s license or passport) to your application. Also provide your school Diploma or GED. You must be 18 or older.

Pay the fee applicable
Provisional Certificate of Registration Fee Payments must be made online in the Mississippi State Department of Health application portal after filing up the details. The Application Fee is $150.
