The Step-Wise Become a Licensed Tattoo Artist in Georgia in 2025
To acquire a License to become a Tattoo Artist, please submit the following to your county’s Environmental Health Office via mail:

Complete License
Application
Fill and Submit a completed Body Art License Application and applicable fee (call your county office for details)

Bloodborne Pathogen
Course
Provide proof of attending an OSHA-compliant bloodborne pathogen /Universal Precautions course by attaching a copy of your certificate to your application. You must have a passing grade of 70%.

Hepatitis B vaccination status
Provide Valid documentation of a Hepatitis B Virus vaccination status including:
- Documentation of HBV vaccination; or
- Laboratory evidence of immunity or documentation of no response following two full HBV vaccine series; or
- Documentation stating the vaccine is contraindicated for medical reasons. Contraindications require a dated and signed licensed health care professional’s statement specifying the name of the Body Artist applicant or employee and that the vaccine cannot be given; or
- Signed certificate of vaccination declination of HBV as required by OSHA.

Basic First Aid
Training Certificate
Provide proof of a Basic First Aid Training course by attaching a copy of your certificate to your application.

Provide a Copy
of a Photo ID
Attach a copy of your photo ID (driver’s license or passport) to your application. You must be 18 or older.

Verification of Residency for Public Benefits Application:
Fill and submit a completed Verification of Residency for Public Benefits Application
Georgia law requires that every permit application submitted to District 4 Public Health Center for Environmental Health be accompanied by a Verification of Residency application to establish that the applicant is lawfully present in the United States of America.
The applicant must present to a Notary Public at least one (1) secure and verifiable identity document along with the affidavit. The original signed and notarized affidavit, along with a copy of the identification document presented to the Notary Public, must be provided to the Center for Environmental Health with the completed application.

Pay the fee applicable
As part of the application process, the applicant shall:
(a) Pay an exam fee not to exceed $50.00 to the Health Authority;
(b) Pay Department Certification fees;
(c) Complete and pass a Department-approved exam.
Every Tattoo Artist also has to pass a Departmental Body Art Exam.
