- Answer Qualifying Questions: You'll encounter a series of qualifying questions on the following page, which require simple yes or no responses.

Take a Headshot only. Make sure to face the camera and that you are not wearing sunglasses or a hat. Ensure a plain background.

Permits must be renewed annually. The fee is usually $25.00. Fees and Renewal period may vary from county to county.

Application form
Submit your completed application on the portal. Typically, it takes around 7 days for the office to process and approve your application. If any additional information is needed, they'll reach out to you via email. Upon approval, you'll receive a temporary badge via email, allowing you to start tattooing immediately at a licensed body art establishment. No waiting is necessary! Once your permanent badge is ready, you have two options:
- Mail Delivery: It can be conveniently mailed to you.
- In-Person Pickup: Alternatively, you can choose to pick it up yourself. Simply specify your preference when submitting your forms and identification.
